SOCCER IN SPANISH ABROAD PARTICIPANT SIGN UP

HOW TO CONFIRM PARTICIPATION

Once Soccer in Spanish receives this form, you will be asked to sign the Terms and Conditions (one per family) and Participant Agreements (one for each family member). Please submit these forms along with the program deposit ($250 per child participant) no later than November 1, 2025.

All forms and the program deposit check can be mailed to: Soccer in Spanish LLC, PO Box 1773, Hoboken, NJ 07030 or they can be dropped off at the Allen-Stevenson front desk under the name of Jesús Parra.

Once these forms and the program deposit check are received, you will receive an email invoice from Soccer in Spanish that states the balance of your program fees, which will depend on the number of child participants. This program balance is due no later than December 19, 2025. Your participation in the Program will not be confirmed until we have received your full payment. You will receive an email confirmation upon receipt of payment.

You will also be asked to fill out a health form and a medical form (filled out by a medical profession) for each family member, which are due no later than January 30, 2026.

Fee Schedule

Program Deposit: $250 per child participant – due no later than November 1, 2025. NOTE: This is not an additional fee. This program deposit will be subtracted from the balance of your total.

Program Fee Balance: Amount varies depending on number of participants – due no later than December 19, 2025.
